ABSTRACT

ABSTRACT Objective: to assess the epidemiological profile of patients undergoing exploratory trauma laparotomy based on severity and prognosis criteria, and to determine the predictive accuracy of trauma scoring systems in terms of morbidity and mortality. Methods: retrospective cohort study and review of medical records of patients undergoing exploratory laparotomy for blunt or penetrating trauma at the Hospital de Pronto Socorro de Porto Alegre, from November 2015 to November 2019. Demographic data, mechanism of injury, associated injuries, physiological (RTS and Shock Index), anatomical (ISS, NISS and ATI) and combined (TRISS and NTRISS) trauma scores, intraoperative findings, postoperative complications, length of stay and outcomes. Results: 506 patients were included in the analysis. The mean age was 31 ± 13 years, with the majority being males (91.3%). Penetrating trauma was the most common mechanism of injury (86.2%), predominantly by firearms. The average RTS at hospital admission was 7.5 ± 0.7. The mean ISS and NISS was 16.5 ± 10.1 and 22.3 ± 13.6, respectively. The probability of survival estimated by TRISS was 95.5%, and by NTRISS 93%. The incidence of postoperative complications was 39.7% and the overall mortality was 12.8%. The most accurate score for predicting mortality was the NTRISS (88.5%), followed by TRISS, NISS and ISS. Conclusion: the study confirms the applicability of trauma scores in the studied population. The NTRISS seems to be the best predictor of morbidity and mortality.


RESUMO Objetivo: analisar o perfil epidemiológico dos pacientes submetidos a laparotomia exploradora por trauma com base em critérios de gravidade e prognóstico, e determinar a acurácia preditiva dos escores de trauma em termos de morbimortalidade. Métodos: estudo de coorte retrospectiva e revisão de prontuários dos pacientes submetidos a laparotomia exploradora por trauma contuso ou penetrante no Hospital de Pronto Socorro de Porto Alegre no período de novembro de 2015 a novembro de 2019. Foram avaliados dados demográficos, mecanismo do trauma, lesões associadas, índices fisiológicos (RTS e Shock Index), anatômicos (ISS, NISS e ATI) e mistos (TRISS e NTRISS), achados intraoperatórios, complicações pós-operatórias, tempo de internação e desfecho. Resultados: foram incluídos 506 pacientes na análise. A idade média foi de 31 ± 13 anos, com predomínio do sexo masculino (91,3%). O trauma penetrante foi o mecanismo de lesão mais comum (86,2%), sendo a maioria por arma de fogo. A média do RTS na admissão hospitalar foi 7,5 ± 0,7. A média do ISS e do NISS foi 16,5 ± 10,1 e 22,3 ± 13,6, respectivamente. A probabilidade de sobrevida estimada pelo TRISS foi de 95,5%, e pelo NTRISS de 93%. A incidência de complicações pós-operatórias foi de 39,7% e a mortalidade geral de 12,8%. O escore com melhor acurácia preditiva foi o NTRISS (88,5%), seguido pelo TRISS, NISS e ISS. Conclusões: o estudo confirma a aplicabilidade dos escores de trauma na população em questão. O NTRISS parece ser o sistema com melhor acurácia preditiva de morbimortalidade.

ABSTRACT

INTRODUCCIÓN: El traumatismo torácico (TT) es una causa importante de morbilidad y mortalidad, presente en el 25-50% de la mortalidad por traumatismo. El TT contuso (TTC) es el tipo más frecuente de TT según las diferentes publicaciones internacionales. OBJETIVO: Nuestros objetivos son describir las características, tratamientos, morbilidad y mortalidad en pacientes hospitalizados por TTC en nuestra institución. MATERIAL Y MÉTODOS: Estudio descriptivo transversal desde enero-1981 a diciembre-2017. Revisión de una base de datos prospectiva, protocolos quirúrgicos y fichas clínicas. Se describen y comparan las características de los TTC. Se calcularon índices de gravedad de traumatismo (IGT): Injury Severity Score (ISS), Revised Trauma Score (RTS-T), Trauma Injury Severity Score (TRISS). RESULTADOS: Total 4.163 pacientes hospitalizados por TT, 1.719 (41,3%) TTC. Hombres 1.327 (77,2%), edad promedio 46,7±18,8 años. Se consideró TT aislado 966 (56,2%), asociado a lesiones extratorácicas 753 (43,8%) y de estos 508 (29,6%) eran politraumatizados. Mecanismo: Accidente de tránsito 838 (48,7%), caída de altura 279 (16,2%). Lesiones y hallazgos torácicos: fractura costal 1.294 (75,3%), neumotórax 752 (43,1%). Tratamiento: médico 874 (50,8%), pleurotomía 704 (41%) y cirugía torácica 141 (8,2%). Período de hospitalización 9,2 ± 9,5 días. Según IGT: ISS promedio 14,1 ± 11,1, RTS-Tpromedio 11,5 ± 1,5, TRISS promedio 6,6. Morbilidad en 297 (17,3%), mortalidad en 68 (4%). DISCUSIÓN: La causa principal de los TTC fue el accidente de tránsito. La fractura costal correspondió a la lesión torácica más frecuente. La mayoría requirió solo tratamiento médico. La mortalidad fue menor a la esperada según IGT.


BACKGROUND: Thoracic trauma (TT) is a major cause of morbimortality, involved in 25-50% of trauma deaths. Internationally, blunt thoracic trauma (BTT) is the most frequent type of TT. OBJECTIVE: Our objectives are to describe the clinical characteristics, treatments, morbidity and mortality in patients hospitalized by blunt thoracic trauma (BTT) in our institution. MATERIAL AND METHODS: Cross-sectional descriptive study from january-1981 to december-2017. Prospective database review, surgical protocols and clinical files. The characteristics of the BTT are described and compared. The following trauma severity indices (TSI) were calculated: Injury Severity Score (ISS), Revised Trauma Score (RTS-T) and Trauma Injury Severity Score (TRISS). RESULTS: 4,163 patients were hospitalized because of TT, 1.719 (41.3%) of them with BTT. 1,327 (77.2%)patients were men, average age 46.7 ± 18.8 years-old. We considered isolated TT 966 (56.2%), associated with extrathoracic lesions 753 (43,8%) and 508 (29.6%)with polytraumatism. Mechanism: Traffic accident 838 (48.7%), fall down from a height 279 (16.2%). Lesions and intrathoracic findings: rib fracture 1.294 (75.3%), pneumothorax 752 (43.7%). Treatment: Medical 876 (50.8%), pleurotomy 704 (41%) and thoracic surgery 141 (8.2%). Average hospitalized period 9.2 ± 9.5 days. According to TSI: ISS average 14.1 ± 11.1, RTS-T average 11.5 ± 1.5, TRISS average 6.6. Morbidity in 297 (17.3%), mortality in 68 (4%). DISCUSSION: The TTC was mainly attributed to the traffic accident. Rib fracture was the most common chest injury. The majority of patients required only medical treatment. Mortality was lower than expected according to TSI.

Objective : to evaluate the epidemiological variables and diagnostic and therapeutic modalities related to hepatic trauma patients undergoing laparotomy in a public referral hospital in the metropolitan region of Vitória-ES. Methods : we conducted a retrospective study, reviewing charts of trauma patients with liver injuries, whether isolated or in association with other organs, who underwent exploratory laparotomy, from January 2011 to December 2013. Results : We studied 392 patients, 107 of these with liver injury. The male: female ratio was 6.6 : 1 and the mean age was 30.12 years. Penetrating liver trauma occurred in 78.5% of patients, mostly with firearms. Associated injuries occurred in 86% of cases and intra-abdominal injuries were more common in penetrating trauma (p <0.01). The most commonly used operative technique was hepatorrhaphy and damage control surgery was applied in 6.5% of patients. The average amounts of blood products used were 6.07 units of packed red blood cells and 3.01 units of fresh frozen plasma. The incidence of postoperative complications was 29.9%, the most frequent being infectious, including pneumonia, peritonitis and intra-abdominal abscess. The survival rate of patients suffering from blunt trauma was 60%, and penetrating trauma, 87.5% (p <0.05). Conclusion : despite technological advances in diagnosis and treatment, mortality rates in liver trauma remain high, especially in patients suffering from blunt trauma in relation to penetrating one.


Objetivo : avaliar as variáveis epidemiológicas e as modalidades diagnósticas e terapêuticas relacionadas ao trauma hepático de pacientes submetidos à laparotomia exploradora em um hospital público de referência da Região Metropolitana de Vitória-ES. Métodos: estudo retrospectivo de revisão de prontuários dos pacientes vítimas de trauma com lesão hepática isolada ou associada a outros órgãos, submetidos à laparotomia exploradora, no período de janeiro de 2011 a dezembro de 2013. Resultados: foram estudados 392 pacientes submetidos à laparotomia, dos quais 107 com lesões hepáticas. A relação masculino:feminino foi 6,6:1 e a média de idade dos pacientes foi 30,12 anos. O trauma hepático penetrante ocorreu em 78,5% dos pacientes, principalmente por arma de fogo. Lesões associadas ocorreram em 86% dos casos e as lesões intra-abdominais foram mais comuns no trauma penetrante (p<0,01). A técnica operatória mais utilizada foi a hepatorrafia, e a cirurgia para controle de danos foi feita em 6,5% dos pacientes. A quantidade média de hemoderivados utilizados foi 6,07 unidades de hemoconcentrado e 3,01 unidades de plasma fresco. A incidência de complicações pós-operatórias foi 29,9%, e as mais frequentes foram as infecciosas, incluindo pneumonia, peritonite e abscesso intra-abdominal. A taxa de sobrevida dos pacientes acometidos de trauma contuso foi 60% e de trauma penetrante, 87,5% (p<0,05). Conclusão: apesar dos avanços tecnológicos de diagnósticos e tratamentos, as taxas de morbimortalidade nos traumas hepáticos permanecem elevadas, especialmente nos pacientes acometidos de trauma hepático contuso em relação ao trauma penetrante.

ABSTRACTObjective:to compare the frequency and the severity of diagnosed injuries between pedestrians struck by motor vehicles and victims of other blunt trauma mechanisms.Methods:retrospective analysis of data from the Trauma Registry, including adult blunt trauma patients admitted from 2008 to 2010. We reviewed the mechanism of trauma, vital signs on admission and the injuries identified. Severity stratification was carried using RTS, AIS-90, ISS e TRISS. Patients were assigned into group A (pedestrians struck by motor vehicle) or B (victims of other mechanisms of blunt trauma). Variables were compared between groups. We considered p<0.05 as significant.Results:a total of 5785 cases were included, and 1217 (21,0%) of which were in group A. Pedestrians struck by vehicles presented (p<0.05) higher mean age, mean heart rate upon admission, mean ISS and mean AIS in head, thorax, abdomen and extremities, as well as lower mean Glasgow coma scale, arterial blood pressure upon admission, RTS and TRISS. They also had a higher frequency of epidural hematomas, subdural hematomas, subarachnoid hemorrhage, brain swelling, cerebral contusions, costal fractures, pneumothorax, flail chest, pulmonary contusions, as well as pelvic, superior limbs and inferior limbs fractures.Conclusion:pedestrian struck by vehicles sustained intracranial, thoracic, abdominal and extremity injuries more frequently than victims of other blunt trauma mechanism as a group. They also presented worse physiologic and anatomic severity of the trauma.


RESUMOObjetivo:comparar, entre vítimas de atropelamento e de outros mecanismos de trauma fechado, a frequência e gravidade das lesões identificadas.Métodos:análise retrospectiva das informações do registro de trauma, incluindo as vítimas de trauma fechado, com idade superior a 13 anos de idade, admitidas entre 2008-2010. Avaliamos o mecanismo de trauma, dados vitais à admissão e lesões diagnosticadas. A estratificação de gravidade da amostra foi realizada pelos índices RTS, AIS-90, ISS e TRISS. As vítimas de trauma fechado foram separadas em dois grupos: Grupo A- pedestres vítimas de atropelamento; Grupo B- vítimas dos demais mecanismos de trauma fechado. As variáveis foram comparadas entre os dois grupos.Resultados:foram incluídos no estudo 5785 casos, sendo que, 1217 (21,0%) foram vítimas de atropelamento. Observamos que os traumatizados do grupo A apresentaram, significativamente (p<0,05), maior média etária, de frequência cardíaca à admissão, de ISS, de AIS no segmento cefálico, torácico, abdominal e em extremidades, bem como, menor média de escala de coma de Glasgow, pressão arterial sistólica a admissão, RTS e TRISS. As vítimas de atropelamento também apresentaram (p<0,05), maior frequência de hematomas extradurais, hematomas subdurais agudos, hemorragia subaracnoidea, Brain Swelling, contusão cerebral, lesão axonal difusa, fraturas de arcos costais, pneumotórax, tórax flácido, contusão pulmonar, bem como, fraturas de pelve, de membros superiores, inferiores e expostas de membros inferiores.Conclusão:as vítimas de atropelamento apresentam maior frequência e gravidade de lesões intracranianas, torácicas, abdominais e em extremidades quando comparadas às vítimas de outros mecanismos de trauma fechado em conjunto.

OBJETIVO: comparar as características do trauma entre idosos e "superidosos". MÉTODOS: análise retrospectiva dos protocolos de vítimas de trauma fechado com idade igual ou superior a 70 anos. Os idosos de idade entre 70 e 79 anos foram incluídos no grupo I, os de idade igual ou maior de 80 anos no grupo II. Análise estatística foi realizada através dos testes t de Student, qui-quadrado e Fisher, considerando p<0,05 significativo. RESULTADOS: Foram incluídos no estudo 281 doentes (grupo I-149; grupo II-132). A idade variou de 70 a 99 anos (79,1 ± 6,7 anos), sendo 52,3% do sexo masculino. Os superidosos se caracterizaram por apresentar menor média de AIS em abdome (0,10 ± 0,59 vs. 0,00 ± 0,00; p=0,029), menor frequência de vítimas do sexo masculino (59,1% vs. 44,3%; p=0,013), maior frequência de queda da própria altura (44,3% vs. 65,2%; p=0,028) e menor frequência de fraturas de membros superiores (9,4% vs. 2,3%; p=0,010). Não observamos diferença significativa na comparação das demais variáveis entre os grupos. CONCLUSÃO: O trauma em idosos é um grave problema de saúde pública, com tendência à piora progressiva pelo envelhecimento da população. Os dados deste estudo nos auxiliam com uma visão mais clara do trauma nos superidosos, um subgrupo que merece atenção especial.


OBJECTIVE: To compare the characteristics of trauma in the elderly under and over 80-years-old. METHODS: We conducted a retrospective analysis of protocols of blunt trauma victims aged over 70 years. Individuals aged between 70 and 79 years were included in group I, those aged 80 years or greater in group II. Statistical analysis was performed using the Student t, chi-square and Fisher tests, considering p <0.05 as significant. RESULTS: The study included 281 patients (group I-149, group II-132). The age ranged between 70 and 99 years (79.1 + 6.7 years), with 52.3% male. Group II had lower average abdomen AIS (0.10 + 0.59 vs. 0.00 + 0.00, p = 0.029), lower incidence of male victims (59.1% vs. 44 , 3%, p = 0.013), higher incidence of falling from standing height (44.3% vs. 65.2%, p = 0.028) and lower frequency of upper limb fractures (9.4% vs. 2.3 %, p = 0.010) than Group I. There was no significant difference in comparison of other variables between the groups. CONCLUSION: Trauma in the elderly is a serious public health problem, with a tendency to progressive worsening with population aging. Data from this study help us with a clearer picture of trauma in the over-80 elders, a subgroup that deserves special attention.

ABSTRACT

OBJETIVO: avaliar os aspectos epidemiológicos, conduta, morbidade e resultados do tratamento trauma hepático. MÉTODOS: estudo retrospectivo de doentes com mais de 13 anos de idade admitidos em um hospital universitário de 1990 a 2010, submetidos ao tratamento cirúrgico ou não operatório (TNO). RESULTADOS: foram admitidos 748 pacientes com trauma hepático. O mecanismo de trauma mais frequente foi o trauma penetrante (461 casos; 61,6%). O trauma fechado ocorreu em 287 pacientes (38,4%). De acordo com o grau de lesão hepática (AAST-OIS), no trauma fechado foi observada uma predominância dos graus I e II e no trauma penetrante, uma predominância do grau III. O TNO foi realizado em 25,7% dos pacientes com trauma hepático contuso. Entre os procedimentos cirúrgicos, a sutura foi realizada com maior frequência (41,2%). A morbidade relacionada ao fígado foi 16,7%. A taxa de sobrevida para pacientes com trauma hepático fechado foi 73,5% e no trauma penetrante de 84,2%. A mortalidade no trauma complexo foi 45,9%. CONCLUSÃO: o trauma permanece mais incidente nas populações mais jovens e no sexo masculino. Houve uma redução dos traumas hepáticos penetrantes. O TNO se mostrou seguro e efetivo, e, frequentemente, foi empregado para tratar os pacientes com trauma hepático penetrante. A morbidade foi elevada e a mortalidade foi maior em vítimas de traumas contusos e em lesões hepáticas complexas.


OBJECTIVE: To evaluate the epidemiological aspects, behavior, morbidity and treatment outcomes for liver trauma. METHODS: We conducted a retrospective study of patients over 13 years of age admitted to a university hospital from 1990 to 2010, submitted to surgery or nonoperative management (NOM). RESULTS: 748 patients were admitted with liver trauma. The most common mechanism of injury was penetrating trauma (461 cases, 61.6%), blunt trauma occurring in 287 patients (38.4%). According to the degree of liver injury (AAST-OIS) in blunt trauma we predominantly observed Grades I and II and in penetrating trauma, Grade III. NOM was performed in 25.7% of patients with blunt injury. As for surgical procedures, suturing was performed more frequently (41.2%). The liver-related morbidity was 16.7%. The survival rate for patients with liver trauma was 73.5% for blunt and 84.2% for penetrating trauma. Mortality in complex trauma was 45.9%. CONCLUSION: trauma remains more common in younger populations and in males. There was a reduction of penetrating liver trauma. NOM proved safe and effective, and often has been used to treat patients with penetrating liver trauma. Morbidity was high and mortality was higher in victims of blunt trauma and complex liver injuries.

OBJETIVO: Identificar fatores preditivos de lesões abdominais em vítimas de trauma fechado. MÉTODOS: Análise retrospectiva dos dados das vítimas de trauma fechado com idade superior a 13 anos, em um período de 15 meses. Comparamos as variáveis entre os doentes com lesões abdominais diagnosticadas por tomografia computadorizada e/ou laparotomia - grupo I (Abbreviated Injury Scale abdome>0, grupo I) e os demais - grupo II (Abbreviated Injury Scale abdome=0,). RESULTADOS: Foram incluídos 3783 casos, com média etária de 39,1 +17,7 anos (14 a 99 anos), sendo 76,1% do sexo masculino. Foram identificadas lesões abdominais em 130 doentes (3,4%). Os traumatizados com lesões abdominais apresentaram, significativamente, menor média etária (35,4 + 15,4 anos vs. 39,2 + 17,7 anos), menor média da pressão arterial sistólica à admissão (114,7 + 32,4mmHg vs. 129,1 + 21,7mmHg), menor média na escala de coma de Glasgow à admissão (12,9 + 3,9 vs. 14,3 + 2,0), maior média de AIS em segmento cefálico (0,95 + 1,5 vs. 0,67 + 1,1), maior média de AIS em segmento torácico (1,10 + 1,5 vs. 0,11 + 0,6) e maior média de AIS em extremidades (1,70 ± 1,8 vs. 1,03 ± 1,2). Os maiores Odds ratio foram presença de tórax flácido (21,8) e fraturas de pelve (21,0). CONCLUSÃO: As lesões abdominais foram mais frequentemente observadas nos doentes com instabilidade hemodinâmica, alteração na escala de coma de Glasgow, lesões graves em crânio, tórax ou extremidades.


OBJECTIVE: To identify predictors of abdominal injuries in victims of blunt trauma. METHOD: retrospective analysis of trauma protocols (collected prospectively) of adult victims of blunt trauma in a period of 15 months. Variables were compared between patients with abdominal injuries (AIS>0) detected by computed tomography or/and laparotomy (group I) and others (AIS=0, group II). Student's t, Fisher and qui-square tests were used for statistical analysis, considering p<0.05 as significant. RESULTS: A total of 3783 cases were included, with a mean age of 39.1 ± 17.7 years (14-99), 76.1% being male. Abdominal injuries were detected in 130 patients (3.4%). Patients sustaining abdominal injuries had significantly lower mean age (35.4 + 15.4 vs. 39.2 + 17.7), lower mean systolic blood pressure on admission (114.7 + 32.4 mmHg vs. 129.1 + 21.7 mmHg), lower mean Glasgow coma scale (12.9 + 3.9 vs. 14.3 + 2.0), as well as higher head AIS (0.95 + 1.5 vs. 0.67 + 1.1), higher thorax AIS (1.10 + 1.5 vs. 0.11 + 0.6) and higher extremities AIS (1.70 ± 1.8 vs. 1.03 ± 1.2). Patients sustaining abdominal injuries also presented higher frequency of severe injuries (AIS>3) in head (18.5% vs. 7.9%), thorax (29.2% vs. 2.4%) and extremities (40.0% vs. 13.7%). The highest odds ratios for the diagnosis of abdominal injuries were associated flail chest (21.8) and pelvic fractures (21.0). CONCLUSION: Abdominal injuries were more frequently observed in patients with hemodynamic instability, changes in Glasgow coma scale and severe lesions to the head, chest and extremities.

ABSTRACT

The blunt trauma injury is the most common type, a pathologist encounter when doing medico-legal autopsies. Sometimes, while searching the main cavities of the body for the fatal injuries like lacerations and damages to the vital organs, we, the Forensic Pathologist commonly ignores the superficial small abrasions, bruises over the other parts of the body. Cutaneous injuries i.e. abrasions, contusions in most cases, are not fatal and are seemingly unimportant. However, soft tissue injuries not always are simple as mostly described in the literature but sometimes these injuries may be so extensive internally that leads to irreversible shock. In fact, these injuries prove fatal when taken collectively into consideration. Sometimes trivial circumstances lead to serious quarrel, resulting into death of the victim which apparently looking uninjured externally while conducting autopsy. Under such circumstances sometimes at primary health care centres the inexperienced autopsy surgeons are not in a position to correlate the superficial looking soft tissue injuries attributing towards death.

ABSTRACT

FREEDOM FROM VIOLENCE, not necessarily proportional with development in some basic capabilities like nutrition, longevity and literacy which positively related to economic growth. So, dealing with violence in society is intrinsically a matter of public interest. In present study 79 cases of homicidal deaths due to different mechanical injuries was studied in two year period from 1/1/2009 to 31/12/2010. The incidence of homicidal death was 9.13% during this period. Males affected more in comparison to females. Majority of cases were fall in 21-40 yrs. of age comprising 50 cases (63.28%).Injuries by blunt weapons commonly observed in 30 cases (37.97%) which followed by injuries by sharp weapons 19 cases (24.05%). Totally 329 sharp injuries observed in comparison to 152 blunt injuries. Incidences were prevalent at dense forest area 26 cases (32.91%), in married persons 55 cases (69.62%), in between 6 pm to 12 midnight 24 cases (30.37%) and revenge was main motive 23 cases (29.11%).

ABSTRACT

OBJETIVO: Realizar uma análise comparativa entre as lesões encontradas em motociclistas envolvidos em acidentes de trânsito e vítimas de outros mecanismos de trauma fechado. MÉTODOS: Análise dos protocolos (colhidos prospectivamente) dos traumatizados com idade superior a 13 anos, admitidos de 10/06/2008 a 01/09/2009, vítimas de trauma fechado. Foram coletadas informações sobre mecanismo de trauma, dados vitais à admissão, exames complementares, lesões e tratamento. A estratificação da gravidade do trauma e das lesões foi realizada pelo cálculo dos índices de trauma: RTS, escala de coma de Glasgow (ECG), AIS, ISS e TRISS. Comparamos as variáveis entre os motociclistas (grupo A) e os demais (grupo B). Consideramos graves as lesões com AIS > 3. Para a análise estatística, utilizamos os testes t de Student, Mann Whitney, qui-quadrado e Fisher, considerando p < 0,05 significativo. RESULTADOS: Foram incluídos no estudo 3.783 vítimas de trauma fechado, com idade de 14 a 99 anos, sendo 76,0 por cento do sexo masculino. Os mecanismos de trauma mais frequentes foram os acidentes com motociclistas (24,4 por cento) e atropelamentos (22,6 por cento). Na comparação das variáveis entre os grupos A e B, observamos que os motociclistas apresentaram, significativamente, (p < 0,05), menor média etária (28,9 ± 8,5 anosvs. 42,4 ± 18,5 anos) e menor média de AIS em segmento cefálico (0,3 ± 0,9 vs. 0,8 ± 1,1), como também, maior média da ECG à admissão (14,5 ± 1,9vs. 14,2 ± 2,2), de AIS em extremidades (1,5 ± 1,2 vs. 0,9 ± 1,2), de RTS (7,8 ± 0,5 vs. 7,7 ± 0,6) e de TRISS (0,98 ± 0,1 vs. 0,97 ± 0,1). O gênero masculino foi significativamente mais frequente no grupo A (87,9 por centovs. 72,6 por cento). Na comparação das lesões entre os grupos, notamos que os motociclistas apresentaram, significativamente, (p < 0,05), menor frequência de hematomas extradurais (0,6 por cento vs. 2,1 por cento), hematomas subdurais (0,9 por cento vs. 2,1 por cento), hemorragia subaracnóidea (0,9 por cento vs. 2,2 por cento), contusão encefálica (1,2 por cento vs. 3,6 por cento) e lesões graves em crânio (4,8 por centovs. 9,4 por cento), bem como maior frequência de lesão axonal difusa (1,6 por centovs. 0,7 por cento), fraturas de membros superiores (7,9 por centovs. 4,4 por cento), inferiores (7,7 por centovs. 5,2 por cento), e lesões graves em extremidades (20,6 por centovs. 12,6 por cento). CONCLUSÃO: Em comparação às vítimas dos demais mecanismos de trauma, os motociclistas apresentaram menor frequência e gravidade das lesões em segmento cefálico, bem como maior frequência e gravidade das lesões em extremidades.


OBJECTIVE: To conduct a comparative analysis of the lesions found among motorcycle riders involved in traffic accidents and victims of other mechanisms of blunt trauma. METHODS: Analysis of data prospectively collected on protocols for trauma patients older than 13 years, admitted from 06/10/2008 to 09/01/2009, victims of blunt trauma. Data collected included trauma mechanism, vital signs at admission, laboratory tests, injuries, and treatment.Stratification of trauma and lesion severity was performed by calculating the trauma index: Glasgow Coma Scale (GCS), Revised Trauma Score (RTS), Abbreviated Injury Scale (AIS), Injury Severity Score (ISS) and TRISS. We compared the variables between motorcycle riders (group A) and the others (group B). Severe injuries were considered when AIS > 3. For statistical analysis, we used Student's t, Mann Whitney, chi-square and Fisher's test, with p < 0.05 considered statistically significant. RESULTS: The study included 3,783 blunt trauma victims, aged 14 to 99 years, of which 76.0 percent were males. The most frequent trauma mechanisms were accidents involving motorcycle riders (24.4 percent) and pedestrians (22.6 percent). When comparing the variables between groups A and B, we observed that motorcycle riders were significantly (p < 0.05) younger (28.9 ± 8.5 years vs. 42.4 ± 18.5 years) and had lower mean AIS in the head segment (0.3 ± 0.9 vs. 0.8 ± 1.1), as well as higher mean GCS at admission (14.5 ± 1.9 vs. 14.2 ± 2.2), AIS in the extremities (1.5 ± 1.2 vs. 0.9 ± 1.2), RTS (7.8 ± 0.5 vs. 7.7 ± 0.6) and TRISS (0.98 ± 0.1 vs. 0.97 ± 0.1). Men were significantly more frequent in group A (87.9 percent vs. 72.6 percent). When comparing injuries between groups, we observed that the motorcycle riders had significantly (p < 0.05) lower frequency of extradural hematomas (0.6 percent vs. 2.1 percent), subdural hematomas (0.9 percent vs. 2.1 percent), subarachnoid hemorrhage (0.9 percent vs. 2.2 percent), brain injury (1.2 vs. 3.6 percent), and severe head injuries (4.8 percent vs. 9.4 percent ), as well as higher frequency of diffuse axonal injury (1.6 percent vs. 0.7 percent), upper- (7.9 percent vs. 4.4 percent) and lower-limb fractures (7.7 percent vs. 5, 2 percent), and severe extremity lesions (20.6 percent vs. 12.6 percent). CONCLUSION: Compared to victims of other trauma mechanisms, motorcycle riders had a lower frequency and severity of head injuries, as well as increased frequency and severity of lesions in the extremities.

ABSTRACT

The present research aims to highlight the pattern of thoraco-abdominal injuries sustained by the victims of fatal road traffic accidents (RTA) in Manipal, South India. The study is an autopsy based observation of thoraco-abdominal injuries in victims of fatal road traffic accidents during 1999 – 2003. Road traffic accidents accounted for most of the injury related deaths (77%) during the study period. A male preponderance (86%) was observed with a male-female ratio of 6:1. Individuals in the age group of 21 to 50 years formed the most vulnerable (83%) group. External thoracic injuries were more common than internal thoracic injuries in the thoracic region. In the abdominal region, internal injuries were more common than external injuries. Lungs (61%) and kidneys (23%) were the most commonly involved organs in the thoracic and abdominal regions respectively. Majority of the victims were two wheeler occupants (35%) followed by pedestrians (23%). The study indicates the pattern of thoraco-abdominal injuries sustained along with the trend of road traffic accidents in the region.

OBJETIVO: avaliar se a presença de fratura de pelve é associada à maior gravidade e pior prognóstico em vítimas de trauma fechado. MÉTODOS: análise retrospectiva dos protocolos e prontuários das vítimas de trauma fechado admitidas de 10/06/2008 a 10/03/2009, separadas em dois grupos: com fratura de pelve (Grupo I) e os demais (Grupo II). Foram avaliados dados do pré-hospitalar e admissão, índices de trauma, exames complementares, lesões diagnosticadas, tratamento e evolução. Utilizamos os testes t de Student, Fisher e qui-quadrado na análise estatística, considerando p<0,05 como significativo. RESULTADOS: No período de estudo, 2019 politraumatizados tiveram protocolos preenchidos, sendo que 43 (2,1 por cento) apresentaram fratura de pelve. Os doentes do grupo I apresentaram, significativamente, menor média de pressão arterial sistêmica à admissão, maior média de frequência cardíaca à admissão, menor média da escala de coma de Glasgow, maior média nos AIS em segmentos cefálico, torácico, abdominal e extremidades, bem como, maior média do ISS e menor média de RTS e TRISS. O grupo I apresentou, com maior frequência, hemorragia subaracnoidea traumática (7 por cento vs. 1,6 por cento), trauma raquimedular (9 por cento vs. 1 por cento), lesões torácicas e abdominais, bem como necessidade de laparotomias (21 por cento vs. 1 por cento), drenagem de tórax (32 por cento vs. 2 por cento) e controle de danos (9 por cento vs. 0 por cento). As complicações foram mais frequentes no grupo I: SARA (9 por cento vs. 0 por cento), choque persistente (30 por cento vs. 1 por cento), coagulopatia (23 por cento vs. 1 por cento), insuficiência renal aguda (21 por cento vs. 0 por cento) e óbito (28 por cento vs. 2 por cento). CONCLUSÃO: a presença de fratura de pelve é um marcador de maior gravidade e pior prognóstico em vítimas de trauma fechado.


OBJECTIVE: To assess whether the presence of a pelvic fracture is associated with greater severity and worse prognosis in victims of blunt trauma. METHODS: A retrospective analysis of protocols and records of victims of blunt trauma admitted from June 2008 to March 2009 was separated into two groups: those with pelvic fracture (Group I) and those without it (Group II). Data were collected from pre-hospital admission rates of trauma, laboratory tests, diagnosed lesions, treatment and outcome. We used the Student t test, Fisher's exact test and chi-square test for statistical analysis, considering p <0.05 as significant. RESULTS: During the study period, 2019 individuals had multiple trauma protocols completed, of which 43 (2.1 percent) had pelvic fractures. Patients in Group I had significantly lower average blood pressure, higher mean heart rate, lower mean Glasgow Coma Scale, the highest average AIS in the segments head, chest, abdomen and extremities, as well as higher mean ISS and lower mean TRISS and RTS on admission. Group I more frequently presented with traumatic subarachnoid hemorrhage (7 percent vs. 1.6 percent), spinal cord injury (9 percent vs. 1 percent), thoracic and abdominal injuries, as well as need for laparotomy (21 percent vs. 1 percent), chest drainage (32 percent vs. 2 percent) and damage control (9 percent vs. 0 percent). Complications were more frequent in group I: ARDS (9 percent vs. 0 percent), persistent shock (30 percent vs. 1 percent), coagulopathy (23 percent vs. 1 percent), acute renal failure (21 percent vs. 0 percent) and death (28 percent vs. 2 percent). CONCLUSION: The presence of a pelvic fracture is a marker of greater severity and worse prognosis in victims of blunt trauma.

ABSTRACT

Objetivo Determinar la exposición laboral accidental a fluidos biológicos por contacto percutáneo en el personal Bioanalista de Laboratorios Clínicos públicos, sus factores asociados y el cumplimiento de medidas post exposición biológica. Métodos Se realizó un estudio descriptivo transversal, la muestra fue de 156 bioanalistas adscritos a laboratorios clínicos del área metropolitana del estado Zulia. Para la recolección de datos se aplicó un instrumento de escalas que exploró la exposición percutánea, los factores vinculados y el cumplimiento de medidas post exposición biológica. Resultados Se evidenció exposición por accidentes percutáneos, representados principalmente por pinchazos y cortaduras, detectados en razón de su ocurrencia en un nivel moderado (media entre 2-3,99). Como factores vinculados a la accidentabilidad percutánea, se registra la ocurrencia en nivel moderado con diversas agujas huecas, con sangre y hemoderivados, en manos y dedos, con una severidad superficial, en áreas de toma y procesamiento de muestras durante el re-encapuchado de objetos punzo cortantes. Un nivel de mediano cumplimiento se obtuvo para el manejo post exposición. Existe relación entre la exposición percutánea con el nivel de cumplimiento detectado para el manejo post exposición p <0,001. Conclusión La magnitud y características de la exposición a fluidos biológicos detectada en este colectivo laboral reviste una problemática que puede impactar en la salud del personal y debe ser abordada institucionalmente para una efectiva gestión de prevención y control de riesgo.


Objective Determining the occupational exposure to biological fluids of medical technicians working in public clinical laboratories caused by accidental percutaneous contact, associated factors and compliance with post-exposure biological measures. Methods This was a descriptive cross-sectional study. The sample consisted of 156 medical technicians assigned to clinical laboratories in the metropolitan area of Zulia state in Venezuela. Data was collected by applying an instrument for exploring exposure and related factors, as well as compliance with established post-biological exposure measures. Results There was evidence of exposure caused by percutaneous accidents, mainly represented by a moderate level of needle-pricks and cuts (2-3.99 mean). There was a moderate level of factors regarding percutaneous injury in the hands and fingers associated with hollow needles, blood and blood products and superficial severity in sample taking and processing areas when recapping needles or handling sharp or cutting objects. A medium level (2-3.99 mean) of compliance was obtained for post-exposure handling. A significant correlation was found (p<001) between percutaneous exposure and level of compliance with post-exposure management. Conclusion The magnitude and characteristics of exposure to biological fluids detected in this work represents a problematic situation which can affect staff health and must be approached by institutions to ensure effective prevention management and risk control.

Introducción: las hernias diafragmáticas traumáticas comúnmente ocurren posteriores a trauma penetrante cerrado. Debido a las lesiones coexistentes y a la naturaleza silenciosa de las lesiones diafragmáticas, el diagnóstico fácilmente se omite. El objetivo de este estudio fue describir las características clínicas de pacientes consecutivos con hernias diafragmáticas traumáticas tratadas quirúrgicamente en nuestra institución. Material y métodos: cohorte prospectiva de pacientes tratados durante un periodo de seis años. Evaluamos edad, sexo, tipo de mecanismo del trauma, tipo de hernia, método diagnóstico, órganos herniados, lesiones asociadas, tiempo de evolución, morbilidad y mortalidad. Los resultados se describen como frecuencias y medidas de tendencia central y de dispersión. Se empleó χ2 y prueba exacta de Fisher para explorar asociación y riesgo. Resultados: se incluyeron 23 pacientes, 19 fueron hombres (82.6 %) y cuatro mujeres (17.4 %). En nueve pacientes (39.1 %) fueron diagnosticadas hernias diafragmáticas agudas y en 14, hernias crónicas (60.9 %). En 18 (78.3 %) el diagnóstico se hizo mediante radiografía de tórax y la mayoría correspondió a hernias crónicas. La historia médica de trauma toracoabdominal cerrado estuvo presente en 95.7 %, en la mayoría relacionado con accidentes vehiculares. Las principales lesiones asociadas fueron trauma encefálico, fracturas costales y contusión pulmonar. La morbilidad se observó en seis pacientes, todos con hernias crónicas. No hubo mortalidad. Conclusiones: la hernia diafragmática traumática es una entidad clínica poco frecuente y constituye un verdadero desafío debido a las dificultades para el rápido y correcto diagnóstico. En etapa crónica hay mayor riesgo para complicaciones tardías como estrangulamiento y necrosis de vísceras.


BACKGROUND: Traumatic diaphragmatic hernias commonly occur after blunt and penetrating trauma. Due to coexisting injuries and the silent nature of the diaphragmatic injuries, the diagnosis is easily missed. We undertook this study to describe the clinical characteristics of consecutive patients with traumatic diaphragmatic hernias treated surgically at our institution. METHODS: We conducted a prospective cohort of patients treated during a 6-year period. We assessed variables such as age, gender, type of mechanism of trauma, type of hernia, methods of diagnosis, herniated organs and associated lesions, time of evolution, morbidity and mortality. Results are described as frequencies, dispersion and central tendency measures. Chi(2) and Fisher's exact tests were used to explore association and risks. RESULTS: Twenty three patients were included, 19 men (82.6%) and 4 women (17.4%). Acute DTH were diagnosed in nine patients (39.1%) and 14 cases presented as chronic DTH (60.9%). In 18 cases (78.3%) the diagnosis was made by chest x-ray and most corresponded to chronic hernias. Medical history of blunt thoracoabominal trauma was present in 95.7% of the cases, most related to car accidents. The principal associated lesions were head injuries, rib fractures and lung contusion. morbidity was observed in six patients, all with chronic hernias. there was no mortality. CONCLUSIONS: DTH is a rare clinical entity and constitutes a true challenge due to difficulties for a rapid and correct diagnosis. In the chronic stage there is an increased risk for late complications such as visceral strangulation and necrosis.

ABSTRACT

Gastrointestinal injuries are the most important among abdominal injuries. These injuries could be the result of blunt trauma, penetrating wound, or iatrogenic 292 gastrointestinal injuries were recorded in our work 13 were iatrogenic, 55 due to blunt trauma and 224 were due to penetrating injuries. These injuries were dealt with according to general surgical rules, any part of the gastrointestinal tract could be injured, no part was immune except the thoracic esophagus which was well protected by the thoracic cage. All patients were subjected to history. clinical examination, diagnostic peritoneal lavage. X-ray and sonogram when possible. Wound infection was 32. burst abdomen 8 D.V.T 12. chest infection 18 minor leak 13. severe leak 8. septicaemia 4 bleeding 3. trauma high risk area 3. pulmonary embolism 3 and mvocardial infarction 2. All gastrointestinal injuries could be treated with favorable outcome except duodenal injuries which were in deep need for meticulous handling and perfect drainage

Computed tomograms of 107 patients of head trauma in the paediatric age group were evaluated. Generalised brain swelling (44%) and falx sign (44%) were the commonest findings observed in the computed tomograms. The overall mortality was 16.8% of which all except one were those with Glasgow coma scale between 3 and 7.

La importancia de los traumatismos torácicos deriva de su alta frecuencia y de su eventual gravedad así como de sus complicaciones cardiovasculares y de sus repercusiones económicas y psicosociales. Se exponen los resultados del estudio retrospectivo de una serie de 359 pacientes con traumatismos torácicos, atendidos en el Hospital San Juan de Dios entre 1988 y 1993. En la casuística se analiza las principales causas de traumatismos torácicos fueron las agresiones y la riñas (83,6 por ciento) relacionadas con la ingesta de alcohol. Las heridas más frecuentes fueron las penetrantes torácicas (87,7 por ciento) con compromiso del espacio pleural, producidas mayoritariamente por armas blancas. Con respecto a tratamiento, en general, bastan medidas simples como la pleurotomía que fue practicada en el 85,7 por ciento de los casos. Sin embargo, el 38,7 por ciento de los pacientes con heridas penetrantes requirieron intervenciones quirúrgicas y el 13,7 por ciento de cirugía torácica. Los datos presentados confirman la frecuencia e importancia de los traumatismos torácicos, su relación con el alcoholismo; su mortalidad de 6,5 por ciento y sus repercusiones económicas tanto individuales y familiares como para el sistema de salud (16 días de hospitalización promedio y 38,7 de intervenciones quirúrgicas)

ABSTRACT

The surgical management of fifteen cases of duodenal trauma is reported. Blunt injury of the abdomen was the commonest mode of injury. Thirteen patients (86.6%) had associated intra-abdominal and/or extra-abdominal injuries. Diagnosis of duodenal injury was made at laparotomy in all patients. In the majority of patients (n = 11), either duodenal repair, or anastomosis with the pylorus or jejunum was done. Duodenal diverticulization or pancreaticoduodenectomy were undertaken in 2 patients each. Feeding jejunostomy was an important adjunct for nutritional support. Septicemia with multisystem organ failure and duodenal fistula were lethal complications. Mortality in the present series was 46.6%.

ABSTRACT

En 122 pacientes consecutivos asistidos se calculó la escala de trauma y el índice de severidad lesional. Los valores hallados fueron en promedio 7,60 y 43,72 para los fallecidos (9,01%) y de 14,60 y 12,05 para los sobrevivientes, siendo ambas diferencias estadísticamente significativas. La posibilidad de sobrevida fue la esperada de acuerdo con el método TRISS. Se concluye que estos índices son útiles para hacer un pronóstico en los pacientes traumatizados y que los resultados obtenidos están acordes a lo esperado según normas internacionales
